There are two
ways you can register for the children's program: 1. Complete and submit the

When, For Whom The
children's program is for children ages 6 months through 12 years old and will
be located at the Queen Elizabeth Fairmont in Montreal, Canada. Snacks and beverages
will be provided. Meals can be supplied by parents or purchased when checking
your child in each day. Medication will NOT be administered by KiddieCorp. Be
sure to fill out the following form COMPLETELY. To ensure a safe and fun-filled
environment, any child who is ill will not be admitted to the children's program.
The cost for the children's program is $10.00 per hour for the first
child and $5.00 per hour, per child for additional children (2-hour minimum required
per child, per day). A Daily Fee (unlimited use) is also available for $80.00
per day, per child.
The
advance registration deadline is October 9, 2009.
Register early as availability is limited and handled on a first-come, first-served
basis. This form must be completed in its entirety and payment received before
registration is confirmed. Cancellations
must be made to KiddieCorp prior to October 9, 2009 for a full refund. Cancellations
made after that date will be subject to a 50% cancellation fee. Once the program
has begun, no refunds will be issued. AS A REMINDER TO ALL TRAVELING
FAMILIES U.S. citizens are now required to present a passport when
traveling to Canada by air. Please note that on June 1, 2009, the U.S. government
will implement the full requirements of the land and sea phase of the Western
Hemisphere Travel Initiative (WHTI). The proposed rules require most U.S. citizens
entering the United States at sea or land ports of entry to have a passport, passport
card, or WHTI-compliant document. Please also note that all children regardless
of age, including newborns and infants, must have their own passport. Don't wait!
